The "Retroactive" Principle (0-3 Months)
Swiss health insurance is mandatory for residents. The law gives you a 3-month grace period to sign up, but coverage always starts retroactively from your arrival date (or permit start date).

You arrive Jan 1st. You enjoy life. You sign up on March 25th.
In April, you will receive a bill for January + February + March + April.

The "Late Enrollment" Penalty (3+ Months)
If you cross the 3-month line, the game changes legally. You are now in "Late Enrollment" (Verspäteter Beitritt).
According to KVG Art. 5, insurers are required to charge a specific penalty if the delay is inexcusable.
- The Fine (Prämienzuschlag): A surcharge of 30% to 50% on your monthly premiums.
- The Duration: You pay this surcharge for twice the duration of your delay. If you are 6 months late, you pay double premiums for 12 months.
The Real Danger: The Coverage Gap
The financial penalty is painful, but the coverage gap is dangerous.
Under "Late Enrollment," your insurance coverage usually starts from the date of signup. You are legally uninsured for the entire period between your arrival and today.
If you had a skiing accident or a hospital visit during that gap, you are personally liable for 100% of the costs. There is no retroactive coverage once you miss the deadline.
How to Minimize the Financial Hit
Strategy 1: Lowest Base Rate
The penalty is a percentage (e.g., 50%). Therefore, 50% of a cheap policy is much less money than 50% of an expensive one.
Tactic: Do not sign up for a premium "Standard" model. Sign up for the absolute cheapest Telmed model available to lower the base for the penalty calculation.
